The Royal Hospital Chelsea and Breaking Barriers Innovations (BBI) would like to invite you to our “Transitions in care: The future of care and wellbeing services for veterans” roundtable discussion.
This roundtable is being convened by Professor the Lord Patel of Bradford OBE, BBI’s chair, and is an opportunity for Commissioners of the Royal Hospital Chelsea and related stakeholders from across the care and treatment sector for veterans in England to explore the changing care and wellbeing needs for veterans – and in particular how an integrated, partnership model for care and wellbeing, such as that provided by the Royal Hospital Chelsea, can reach more veterans in need.
This roundtable will be taking place on Wednesday 27th March from 10.00 to 12.00 at the Royal Hospital Chelsea, and the themes we will be addressing are:

• The changing demographics and needs of veterans
• Service integration – a holistic model for care and wellbeing
• Transitions and partnership – a new approach to community and residential care
