Description of impact
This is a new, community-based health service in Middlesbrough. The aim is to optimise patients' health and wellbeing before they undergo major surgery in order to minimise the risks of surgery and to facilitate a quick return to normal activities. The project, which involves cross-sector working (primary care, secondary care, public health, academia), has initially been funded for 12 months through the Health Foundation's Innovating for Improvement funding stream.Category of impact | Health and welfare, Practitioners and professional services |
